• Eligibility Service - Financial eligibility is available for patients without health

insurance and who need to see a doctor. Services can be provided on a

discounted fee scale, based on your income and the number of persons in your

immediate family. Please schedule an appointment with an eligibility counselor

no less than 24 hours before your medical appointment. Please bring

the following documents to your eligibility appointment.

Identification Needed:

• Valid Driver's License

• Social Security Care and Birth Certificate

• Proof of address (example: electric bill, phone bill, etc.)

• Income Documentation:

• Income tax returns for the past year with W2's and Schedules OR

• Three (3) most recent paycheck stubs for each working member of the family OR

Unemployment check stub or the determination form

As Appropriate:

• Workman's Comp (SSI) Income

• SSI (Supplemental Security) Income

• Child Support/Alimony

• Full Time Student Documentation (verification letter from school with

credit hours and expiration date)
